During the registration process, you will be asked to provide patient information, such as patient medical history and reason for visit. Additional forms may be required at check-in based on visit type. Use the resource links below to prepare for your visit prior to arrival, as needed.  

*Our providers cannot authorize medical clearance for student-athletes to return-to-play if the need for clearance stems from a head injury.
